1. Daughters name
2. Parents name, address,phone number (home & cell) email address.
** She need the address listed to where all correspondents will go (receipts,monthly statements, etc...). This is the address that will receive any information that will go out that is not through email.
*** If you have multiple addresses, please indicate which address to send correspondents to.
